Indemnify & Hold Harmless



I agree to indemnify and hold harmless Gan Torah Preschool, Chabad of Greater South Bay and its officers, employees, volunteers or assigns from any liability concerning my child’s involvement in the preschool program and further agree that the use of any premises during the program is made at my own risk.


Parental consent for participation in all activities of Gan Torah Preschool is fully implied unless specifically excluded during the registration process.



By signing this agreement, I certify that I have legal capacity to act as the parent/guardian of the named minor. I further understand that the terms of this agreement are legally binding and certify that I am signing this agreement, after having carefully read it, of my own free will.



Coronavirus / COVID-19 Guidelines & Risks



COVID-19 Coronavirus is an extremely contagious virus that spreads easily through person-to-person contact. COVID-19 can lead to severe illness, personal injury, permanent disability, and death. Participating in Gan Torah Preschool can increase the risk of contracting COVID-19. While we are taking precautions to mitigate the risks, Gan Torah Preschool in no way warrants that COVID-19 infection will not occur through participation in our preschool program.



I understand that there are many new rules and regulations in child care centers due to the Coronavirus pandemic and that my child will be required to follow strict social distancing, sanitizing and related rules while at preschool. Children who cannot follow these rules will be removed from the program without a refund.



I agree to follow all the health guidelines being introduced at preschool including checking my child's temperature before drop-off each morning and not bringing them in if they are displaying a fever, vomiting, diarrhea or other flu symptoms; or after traveling out of the area or being around someone who may have the virus. I understand that all children will be subjected to a health screening before entering preschool each morning including an electronic temperature check and they will be sent home if a fever is found.



On behalf of myself and my child, I knowingly and freely, assume all such risks, both known and unknown, relating to my child’s participation in Gan Torah Preschool and/or arising from or relating to COVID‐19, and I hereby waive, release, hold harmless and agree not to sue Gan Torah Preschool, Chabad of Greater South Bay, its employees, agents and representatives concerning any liability arising out of or related to COVID-19 or Gan Torah Preschool’s actions or omissions regarding COVID-19 or the pandemic.



Licensing Agency Rights



I am aware that the State of California Licensing Agency (Community Care Licensing) has the following authority:



  1. To interview children or staff and to inspect and audit child or facility records without consent
  2. To observe the physical condition of the children, including conditions which could indicate abuse, neglect or inappropriate placement and to have a Licensed Medical professional physically examine the children



Medical Care



In the event of an emergency, serious illness, or accident, Gan Torah Preschool has permission to arrange for any necessary first-aid or care by a licensed physician for any child/ren attending our preschool. Every effort will be made to contact the parent/guardian and emergency contacts first. Should it be necessary for the well being of the student to utilize outside medical or dental services all expenses involved will be paid for by the parent.



Parents are required to notify Gan Torah Preschool in a timely manner of any infectious diseases or infestations acquired by a student within our program.



Dismissal from our Program



Gan Torah Preschool reserves the right to dismiss, at its sole discretion, any student whose condition, conduct, influence or behavior is deemed unsatisfactory or detrimental to the best interests of the preschool or fellow students or who violates preschool rules and regulations, in which case no refunds will be made.



Images & Rosters



I give permission for Gan Torah Preschool & Chabad of Greater South Bay to use in promoting the preschool and in other ventures directly relating to the preschool including on Facebook (i) digital, photographic, video, and audio images or likenesses of a student; and (ii) statements, articles, names, music, art, photographs, audio recordings, films and videos created by students or originating from preschool or from a preschool-related activity.



I also give permission for our family's names, addresses, phone numbers and email addresses to be included in the student directory and shared with other parents from the school.



Cancellations Policy



Gan Torah requires a 30 day notice before withdrawing from the preschool and ending tuition payments.

 

In case of a school closure due to county health guidelines:



There will be no refunds for short closures of under 30 days or the first 30 days of a longer closure

After 30 days all accounts will be placed on hold until school is able to resume



Please Note: There will be no refunds for illness of a student that prevents him/her from attending our program.
